By completing this proof of purchase form, you will automatically be eligible to receive 3,000 PrimaVac Pouches. Hamilton Beach Commercial pouches are universal, so you can use them on any in-chamber machine, plus they are NSF-approved and BPA-free.   

Upon purchase, please complete this form and attach a copy of your invoice as proof of purchase. Then, simply choose the pouch sizes you would like.  There are 6 choices (all 3 mil thickness), each in cases of 1,000.  So you can choose all the same size, or up to three different sizes.  

As a bonus, by completing this form, you will register the warranty which will give you an additional year, for 3 years total.
